refactor: 优化知识库获取逻辑,提升性能和数据处理效率

- 修改 get_all_databases 方法,避免预加载文件数据,直接构造返回数据,减少查询时间。
- 批量获取集合信息,减少对 Milvus 的单独查询,提高数据获取效率。
